摘要:

目的 摸索建立基于智能药学平台下更年期联合门诊用药管理体系，为推进医院药师开展联合门诊服务模式提供参考依据。方法 以某院更年期联合门诊为例，梳理就诊流程，细化更年期相关药物干预管理流程。将智能药学平台对接医院HIS工作站，构建用药管理体系信息平台，结合更年期相关药物治疗管理方案，建立用药管理数据库。通过平台进行分层干预、专档管理、定期随访，形成连续的、可追踪的信息化管理体系。结果 基于智能药学平台，建立患者用药管理数据库，我院已初步建立更年期联合门诊用药管理体系。借助平台优势，梳理各环节的开发重点，持续做好更年期联合门诊信息化挂你工作，并推进“互联网+”与智能药学平台融合后在联合门诊工作的开展。结论 践行国家关于健康中国战略的重要目标精神，依托平台优化更年期联合门诊工作模式，建立特色化的更年期联合门诊用药管理体系，可创新临床服务模式，可为其他医疗机构提供一定参考依据。

Abstract:

Objective Exploring the establishment of a medication management system for menopausal joint outpatient services based on an intelligent pharmaceutical platform, providing a reference basis for promoting the joint outpatient service model of hospital pharmacists.Methods Taking the menopausal joint outpatient department of a certain hospital as an example, sort out the consultation process and refine the management process of menopausal related drug intervention. Integrate the intelligent pharmacy platform with the hospital"s HIS workstation, construct a medication management system information platform, and combine with menopausal related medication treatment management plans to establish a medication management database. Through the platform for layered intervention, specialized file management, and regular follow-up, a continuous and traceable information management system is formed.Results Based on the intelligent pharmaceutical platform, a patient medication management database has been established, and our hospital has preliminarily established a menopausal joint outpatient medication management system. With the advantages of the platform, we sorted out the development priorities of each link, continued to do a good job in the informatization of the menopausal joint outpatient service, and promoted the development of the joint outpatient service after the integration of "Internet plus" and the intelligent pharmaceutical platform.Conclusion Implementing the important goals and spirit of the national strategy for a healthy China, relying on platforms to optimize the working mode of menopausal joint clinics, establishing a distinctive medication management system for menopausal joint clinics, and innovating clinical service models，it can provide certain reference basis for other medical institutions.
