| 膦甲酸钠是广谱抗病毒药物,是治疗巨细胞病毒的抗病毒药物之一,其不良反应未见有致QT间期延长的报道。本文介绍1例老年重症患者在输注膦甲酸钠氯化钠注射液后出现QT间期明显延长,临床药师与医师团队结合检验结果和患者的不良反应,对患者进行个体化治疗,建议暂停膦甲酸钠,调整给药方案为更昔洛韦抗病毒治疗,以期达到更安全的治疗效果。该病例通过对QT间期延长可能的机制、危险因素及监护管理的分析,以期为临床药师提高相似病例的风险预警意识,从而实施个体化的药学监护,为临床合理用药提供参考。 |

| Fosnate is a broad-spectrum antiviral drug, which is one of the antiviral drugs to treat cytomegalovirus. No adverse reactions have been reported to induce QT interval prolongation. In this case report, a severe elderly patient with significantly prolonged QT interval after infusion of foscarnet. Clinical pharmacists and physician teams combined the test results and adverse reactions of the patient to conduct individualized treatment for the patient. It is recommended to suspend foscarnet and adjust the administration schedule to ganciclovir antiviral therapy in order to achieve safer therapeutic effects. Through the analysis of the possible mechanism, risk factors and monitoring management of QT interval prolongation, this case is expected to improve the risk warning awareness of similar cases for clinical pharmacists, so as to implement individualized pharmaceutical care and provide references for clinical rational drug use. |
