摘要:

为践行“健康中国2030”战略，构建符合中国国情与时代特征的脑卒中全周期管理模式，本研究旨在创建一套基于“关口前移”理念的闭环智慧防治体系。该体系将脑卒中防治阵线前移至颈动脉斑块阶段，通过整合早期筛查、定期随访、危险分层、精准科普、早期诊断与治疗及康复管理，实现全周期闭环式智慧化管理。体系强调多学科协作，强化对卒中高危人群“预防-急救-康复”的全链条管理，以促进患者更好地回归社会与家庭；并通过推进紧密型医联体建设，建立健全脑卒中全周期健康管理规范，为我国脑卒中防控工作提供实践参考。

Abstract:

To implement the "Healthy China 2030" strategy, it is imperative to establish a new, all-round, and full-cycle management model for stroke that aligns with China""s national conditions and contemporary needs. This study focuses on constructing a closed-loop intelligent prevention and treatment system based on the concept of "moving intervention points forward." The system shifts the focus of stroke prevention and control to the carotid artery plaque stage, achieving early screening, regular follow-up, hierarchical management, precise science popularization, early diagnosis, early treatment, and rehabilitation follow-up for high-risk populations through full-cycle closed-loop intelligent comprehensive management. It emphasizes interdisciplinary collaboration to strengthen the full-chain management of "prevention, emergency care, and rehabilitation" for high-risk groups, thereby facilitating better patient reintegration into society and family. Furthermore, it promotes the construction of a tightly-knit medical consortium and establishes sound full-cycle health management standards for stroke, aiming to provide a reference for the development and refinement of stroke management in China.
