参芎葡萄糖注射液联用头孢哌酮钠/舒巴坦治疗AECOPD的随机对照研究

摘要:

目的 观察参芎葡萄糖注射液联用头孢哌酮钠/舒巴坦治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease，AECOPD)的临床疗效和安全性。方法 将确诊的104例Ⅰ、Ⅱ级AECOPD患者随机分为试验组和对照组，各52例。在常规应用吸氧、止咳、解痉平喘、强心、利尿、纠正电解质和酸碱失衡基础上，对照组静脉滴注头孢哌酮钠/舒巴坦，每次2 g，每日2次；试验组在对照组基础上加用参芎葡萄糖注射液100 mL，每日1次，疗程均为7~14 d。治疗后检查2组患者的肺功能、血气分析、生活质量改善情况、细菌清除率，用统计学方法比较2组的差异。结果 试验组临床有效率和细菌清除率分别为88.46%和88.00%，高于对照组的73.08%和66.67%，与对照组相比，试验组可显著改善肺功能、增加氧分压和降低二氧化碳分压，改善生活质量指标，2组比较差异有统计学意义(P均<0.05)。用药期间均无严重不良反应发生。结论 参芎葡萄糖注射液联用头孢哌酮钠/舒巴坦治疗AECOPD临床疗效确切，安全性好。

Abstract:

OBJECTIVE To observe curative effect of Shenxiong Glucose injection jointed with cefoperazone/sulbactam in treatment acute exacerbation of chronic obstructive pulmonary disease(AECOPD). METHODS All of 104 cases of Ⅰ and Ⅱ level of AECOPD patients were randomly divided into experimental group and control group with 52 cases in each group. On the basis of routine use of oxygen, relieving a cough, relieving asthma and spasm, making heart strong, diuresis and correcting electrolyte and acid-base imbalances, the control group was given cefoperazone/sulbactam by intravenous drip, 2 g each time, 2 times a day. The experimental group based on the control group were pluged Shenxiong Glucose injection by intravenous drip, 100 mL each time, once time a day. The total course of treatment was 7-14 d. Checked pulmonary function, blood gas analysis and improve of life quality, the bacterial clearance rate, etc. after treatment in two groups. The differences between two groups were analyzed by statistical method. RESULTS Total effectiveness rate was 88.46% in the treatment group, which was 73.08% in the control group(P<0.05). And the bacterial clearance rates was 88.00% in the treatment group, which was 66.67% in the control group(P<0.05). Compared with the control group, experimental group could significantly improve lung function, increase p(O₂) and reduce p(CO₂), improve the quality of life index(P<0.05). During the treatment there were no serious adverse reactions. CONCLUSION Shenxiong Glucose injection combined with cefoperazone/aulbactam treatment in AECOPD assures curative effects, less complications and less adverse effects.
