摘要: |
目的 了解儿科合理用药调研指标及药物利用情况,为促进儿童合理用药提供参考。方法 对儿科门诊处方进行合理用药调研指标进行评估,并对儿科住院患者重点药物进行药物利用指数分析。结果 儿科门诊处方平均用药2.49种,人均次费用73.13元,抗菌药使用率19.74%,针剂使用率16.08%,基本药物使用率15.15%,患儿平均就诊时间为6.06 min,与药师接触的平均时间为0.63 min,实际调配药物率为97%,药袋标示率及通用名使用率均达100%。住院抗菌药物使用率61.5%,费用占同期药品费用的14.41%,使用频度最高的为头孢曲松钠,万古霉素与氟康唑注射液CDUI>1,单位体质量日剂量偏大。免疫调节剂及中药注射剂消耗金额分别占儿科同期药品的12.19%、0.75%,DUI均<1。结论 儿科门诊处方用药基本合理,但基本药物使用率偏低,患者关怀指标有待一步改善。住院患儿抗菌药使用率偏高,万古霉素与氟康唑注射液使用欠合理。部分药品尚缺少儿童用药合理性评价的数据与标准。 |

OBJECTIVE To explore the selected drug use indicators (SDUI) and drug utilization indicators (DUI) in pediatrics of our hospital and provide a reference for promoting the rational use of medicines for children. METHODS The rational drug use of pediatric outpatient prescriptions were surveyed on SDUI, and the focus drugs in pediatric inpatient were analysed based on drug utilization index. RESULTS The average number of pediatric outpatient prescription drug varieties was 2.49, per capita cost was 73.13 yuan. The respective usage of antibacterials, injecting drug and essential drugs was 19.74%, 16.08% and 15.15%. The average time of the treatment was 6.06 minutes, and average contact time was 0.63 minutes. Actual drug deployment rate was 97%. The drug bag labeled and generic name usage rates reached 100%. Antibiotics usage was 61.5% inpatient and the costs accounted for 14.41% of total drug costs in the same period. The highest frequency of use was ceftriaxone. The CDUI of vancomycin and fluconazole were over 1. The cost of immunomodulators and TCM injections accounted for 12.19%, 0.75%, and the DUI of them were less than 1. CONCLUSION Pediatric outpatient prescriptions are basically reasonable, but utilization of essential drugs was low. Patient care indicators can be further improved. Usage of antimicrobials in inpatient is high, and the usage of vancomycin and fluconazole injection is less reasonable. Some drugs are still lack of data and standards for the evaluation of the rationality of pediatric medication. |
