喹硫平与利培酮治疗中国老年精神分裂症的系统评价

摘要:

目的 比较喹硫平与利培酮治疗中国老年精神分裂症疗效及不良反应。方法 检索国内喹硫平与利培酮治疗老年精神分裂症临床对照研究文献，检索时间为2000年1月—2014年12月。采用系统评价的Peto法对查阅到的12篇文献进行评估。结果 喹硫平与利培酮的疗效无显著性差异(P>0.05，OR_合并=1.2，95%CI：0.90~1.62)，2组药物不良反应比较，失眠、恶心呕吐、肝功能异常差异有统计学意义(P<0.05)，利培酮组不良反应多于喹硫平组；a锥体外系反应、b体质量增加差异具有显著性(P<0.001，^aOR_合并=0.15，95%CI：0.10~0.20，^bOR_合并=0.3，95%CI：0.18~0.50)，利培酮组明显多于喹硫平组，其他不良反应差异无统计学意义(P>0.05)。结论 喹硫平与利培酮治疗中国老年精神分裂症疗效相当，但喹硫平不良反应较轻。

Abstract:

OBJECTIVE To compare the differences in the efficacy and adverse reaction between quetipiapine and risperidone in the treatment of senile schizophrenia in China. METHODS the clinical control study of the quetiapine and risperidone in the treatment of the old schizophrenia was searched from January 2000 to December 2014, and the 12 literatures were evaluated by systematic review. RESULTS There was no significant difference of the efficacy of quetiapine and risperidone (P>0.05, OR=1.2, 95%CI: 0.90-1.62) and adverse drug reactions in the two groups, while insomnia, nausea, vomiting, liver function abnormal differences had statistical significance (P<0.05). The areaction of extrapyramidal system and bincreased weight indicated significantly(P<0.001, ^aOR=0.15, 95%CI: 0.10-0.20, ^bOR=0.3, 95%CI: 0.18-0.50), and other adverse reactions showed no statistical significance(P>0.05). CONCLUSION the efficacy of the quetiapine versus risperidone in the treatment of Chinese senile schizophrenia is fairly, but the adverse reactions of the quetiapine is mild.
