摘要:

目的 探讨鼻敏康汤剂联合依巴斯汀治疗变应性鼻炎的疗效及血清炎症因子水平分析。方法 选取2016年3月—2017年3月门诊变应性鼻炎患者120例，采用随机数字表法和单盲法分为治疗组和对照组各60例。对照组给予口服依巴斯汀片口服，每次1片，每日1次。治疗组在对照组基础上给予中药鼻敏康汤剂，每日2次，每次150 ml。2组患者均连续治疗4周。观察2组患者临床疗效，比较2组患者的症状评分和血清学指标（IL-4、IL-12、IFN-γ、IL-4/IFN-γ、IgE）及复发率的情况。结果 治疗组显效32例，有效20例，总有效率为86.67%；对照组显效14例，有效26例，总有效率为66.67%，2组比较差异有统计学意义（P<0.01）。2组患者单项症状评分和总积分较治疗前均显著降低，治疗组明显优于对照组（P<0.05），差异均具有统计学意义。2组患者血清IgE及IL-4水平较治疗前显著降低，IFN-γ及IL-12水平较治疗前显著升高（P<0.01），治疗组血清学指标改善程度明显优于对照组（P<0.05），差异均具有统计学意义。治疗组3，6个月的复发率分别为5.77%，9.62%，对照组的分别为35%，55%，治疗组复发率显著低于同期对照组，2组比较差异有统计学意义（P<0.01）。结论 鼻敏康汤剂联合依巴斯汀治疗变应性鼻炎可以有效缓解患者的临床症状，改善相关血清炎症因子水平，临床疗效确切，远期疗效好，值得临床推广应用。

Abstract:

OBJECTIVE To explore curative effect of Bimin Kang decoction combined with Ebastine tablets on allergic rhinitis, and to analyze the serum levels of inflammatory factors. METHODS The 120 outpatients were selected from March 2016 to March 2017, and were randomly divided into control group and treatment group, 60 cases in each group. Control group given Ebastine tablet once a day, 10 mg·d^-1. The treatment group was given Bimin Kang decoction on the basis of control group. Two groups were treated for 4 weeks. The curative effect of two groups of patients with symptom scores and serum indicators(IL-4, IL-12, IFN-γ, IL-4/IFN-γ, IgE) and recurrence rate were observed. RESULTS After treatment, in the treatment group 32 cases were markedly effective, 20 cases were effective, the total effective rate was 86.67%; in the control group 14 cases were markedly effective, 26 cases were effective, the total effective rate was 66.67%; the difference was statistically significant (P<0.01). The scores of the individual symptoms and the total scores of the two groups were significantly lower than those before the treatment(P<0.01), and the treatment group was better than the control group, the difference between the two groups was statistically significant (P<0.05). Two groups of patients with serum IgE and IL-4 levels were significantly lower than those before treatment(P<0.01), IL-12, IFN-γ levels were significantly higher than those before treatment(P<0.01), the improvement degree of serum improvement in treatment group was better than that in control group, the difference between two groups was statistically significant(P<0.05). The recurrence rate of the treatment group was 5.77% and 9.62%, after the follow-up of 3 months and 6 months, and the control group were 35% and 55%. The recurrence rate of the treatment group was significantly lower than that of the control group, the difference between the two groups was statistically significant (P<0.01). CONCLUSION Bimin Kang decoction combined with Ebastine tablets on allergic rhinitis has better clinical curative effect, can effectively relieve the clinical symptoms, improve the level of cytokines, and has a good long-term effect, which is worthy of clinical application.
