盐酸倍他司汀注射液致横纹肌损伤1例

Striated Muscle Injury Caused by Betahistine Hydrochloride Injection

摘要:

1例79岁男性患者因良性阵发性位置性眩晕给予盐酸倍他司汀注射液20 mg静脉滴注。首次用药20 min后，患者出现全身乏力、四肢肌肉剧烈疼痛，伴大汗，伴坐立不安、血压升高、心率加快、呼吸急促等症状，考虑为盐酸倍他司汀注射液导致的药物不良反应。医师立即停用该药，并给予吸氧，葡萄糖氯化钠注射液250 ml静脉滴注。3 h后患者症状缓解，肌肉疼痛、乏力等症状基本消失。次日查血示肌酸激酶显著升高。入院第6天复查肌酸激酶较前明显下降。患者出院1个月后随访，无头晕，无肌肉疼痛。

Abstract:

A 79-year-old male patient received intravenous infusion of betahistine hydrochloride 20 mg for benign paroxysmal positional vertigo. Twenty minutes after the first medication, the patient developed general weakness, severe pain in the muscles of the limbs, accompanied by profuse sweating, restlessness, increased blood pressure, rapid heart rate, and shortness of breath, which was considered to be caused by betahistine hydrochloride injection. The drug was stopped immediately by the physician, and oxygen inhalation and 250 ml intravenous infusion of glucose sodium chloride injection were given. After 3 hours, the symptoms such as muscle pain and fatigue basically disappeared. Blood tests the next day showed a significant increase in creatine kinase. On the sixth day after admission, the creatine kinase was significantly lower than before. The patient was followed up 1 month after discharge with no dizziness or muscle pain.
