ncRNA介导的ceRNA调控网络及其靶向治疗在糖尿病肾病中的研究进展

摘要:
糖尿病肾病(DN)作为终末期肾脏疾病的主要原因,已对公众造成严重威胁。然而,ncRNA介导的竞争性内源RNA(ceRNA)网络通过调控DN进展中的主要途径—炎症、凋亡、纤维化、氧化应激、内质网应激和自噬等干预DN进展中的靶标组织—足细胞、系膜细胞和肾小管上皮细胞,已形成了一种多层次调控模式,在DN进展中扮演着重要角色。此外,外泌体ceRNA网络、生物分子及中医药靶向调控ceRNA网络在DN进展中愈发重要。基于此,本综述总结了ceRNA调控网络、调控途径及靶标组织—DN、ceRNA调控网络、外泌体来源的ceRNA调控网络以及靶向调控ceRNA网络在DN中的研究进展。探讨了ceRNA调控网络当前面临的瓶颈,靶向治疗面临的挑战以及中药干预ceRNA网络的药代动力学缺陷等,旨在为继续探究DN的分子机制及药物研发提供新思路。

Research Advances in NcRNA-Mediated ceRNA Regulatory Networks and Their Targeted Therapeutic Strategies for Diabetic Nephropathy

Abstract:
Diabetic Nephropathy, as a primary cause of end-stage renal disease, poses a serious threat to public health. However, the ncRNA-mediated competitive endogenous RNA network has established a multi-level regulatory pattern by intervening in the target tissues involved in DN progression—podocytes, mesangial cells, and renal tubular epithelial cells—through the regulation of key pathways in DN progression, including inflammation, apoptosis, fibrosis, oxidative stress, endoplasmic reticulum stress, and autophagy. This network plays a crucial role in the progression of DN. In addition, the exosome ceRNA network, biomolecules, and traditional Chinese medicine-targeted regulation of the ceRNA network are becoming increasingly important in the progression of DN. Based on this, this review summarises the research progress on ceRNA regulatory networks, regulatory pathways and target tissues—DN, ceRNA regulatory networks, exosome-derived ceRNA regulatory networks, and targeted regulation of ceRNA networks in DN. Furthermore, it discusses current bottlenecks in ceRNA network research, challenges in targeted therapy, and pharmacokinetic deficiencies in Traditional Chinese Medicine interventions targeting ceRNA networks. This synthesis aims to offer novel perspectives for further exploration of DN""s molecular mechanisms and drug development.
