瑞卡西单抗联合他汀治疗非家族性高胆固醇血症成本效用分析

摘要:
目的? 基于国内经济水平及医疗现状,对非家族性高胆固醇血症患者使用瑞卡西单抗注射液进行药物经济学评价。方法? 根据中国心血管事件发生率的观察数据、Remain-2试验亚洲亚组的疗效、中国卫生统计年鉴的成本和效用和在中国发表的研究,研究时限终生,效果以质量调整生命年(QALYs)表示,使用Markov模型来估计增量成本-效果比(ICER)评价瑞卡西单抗注射液经济性,分析单因素敏感性和概率敏感性,验证结果可靠性。结果? 在瑞卡西单抗150 mg q4w全生命周期的增量质量调整生命年(QALY)为1.44,增量成本为59 256 元,增量成本效果比(ICER)为41 097元/QALY;在瑞卡西单抗300 mg q8w 全生命周期的增量质量调整生命年(QALY)为1.49,增量成本为54 186元,增量成本效果比(ICER)为36 436元/QALY;在瑞卡西单抗450 mg q12w全生命周期的增量质量调整生命年(QALY)为1.19,增量成本为59 512元,增量成本效果比(ICER)为50 070元/QALY,三种不同给药方案下ICER均低于1倍人均 GDP 的阈值(95 749元,2024年),相较于单用他汀治疗具有明显经济优势。结论? 在1倍的2024年我国人均GDP下瑞卡西单抗治疗非家族性高胆固醇血症具有明显的经济学优势,且瑞卡西单抗300 mg q8w给药方案经济性最佳。

Abstract:
ABSTRACT: OBJECTIVE To conduct a pharmacoeconomic evaluation of recaticimab injection in patients with non-familial hypercholesterolemia based on China's economic level and medical status. METHODS Based on the observational data of cardiovascular event incidence in China, the efficacy of the Asian subgroup in the Remain-2 trial, the cost and utility data from China Health Statistics Yearbook, and studies published in China, a lifetime period was set. The effect was expressed in quality-adjusted life years (QALYs). A Markov model was used to estimate the incremental cost-effectiveness ratio (ICER) to evaluate the economics of recaticimab injection. Univariate sensitivity analysis and probabilistic sensitivity analysis were performed to verify the reliability of the results. RESULTS For the whole life cycle of Recaticimab 150 mg q4w, the incremental quality - adjusted life year (QALY) is 1.44, the incremental cost is 59 256 yuan, and the incremental cost - effectiveness ratio (ICER) is 41 097 yuan/QALY. For the whole life cycle of Recaticimab 300 mg q8w, the incremental QALY is 1.49, the incremental cost is 54 186 yuan, and the ICER is 36 436 yuan/QALY. For the whole life cycle of Recaticimab 450 mg q12w, the incremental QALY is 1.19, the incremental cost is 59 512 yuan, and the ICER is 50 070 yuan/QALY.The ICERs of the three different administration regimens are all lower than the threshold of 1 time the per capita GDP (95 749 yuan, 2024). Compared with statin monotherapy, they have obvious economic advantages.CONCLUSION Under the threshold of 1 times China's per capita GDP in 2024, Recaticimab shows obvious economic advantages in the treatment of non-familial hypercholesterolemia, and the 300 mg q8w dosing regimen of Recaticimab has the best economic performance.
