| 目的 根提高慢病患者的院外自我健康管理,以达到稳定病情、提高疗效、降低疾病复发或恶化风险的目的。方法 采用药学交叉计算机的方法,利用移动通信结合语音智能互动技术,开发慢病管理医管家系统,详细论述该系统中慢病计划提醒、监测数据记录、互动咨询、科普知识4大模块的功能与应用。结果 该系统通过多学科团队为患者定制简单易用、个性化的院外健康管理计划,以图文声并茂的方式推送至患者,有利于患者及时执行慢病管理计划,并基于主动监护得到完整的监测数据,为调整慢病计划提供依据。经测试该系统运行稳定流畅,可操作性强,通俗易懂、简单易用。结论 通过信息化建设赋能,掌上医管家有望提高慢性病的防控,为实现全程化、智能化、适老化的现代慢病共管服务体系提供支持。 |

| OBJECTIVE To improve the out-of-hospital self-health management of patients with chronic diseases in order to stabilize their conditions, improve the efficacy of treatment, and reduce the risk of disease recurrence or deterioration. METHODS An interdisciplinary approach integrating pharmacy and information technology was adopted to develop a chronic disease management medical housekeeper system using mobile communication combined with voice intelligent interactive technology, discussing in detail the functions and applications of the four modules of the system, namely, chronic disease plan reminder, monitoring data recording, interactive consultation, and scientific knowledge. RESULTS The system customizes easy-to-use and personalized out-of-hospital health management plans for patients through a multidisciplinary team, and pushes them to patients in a graphic-text-voice manner, which is conducive to the timely implementation of the chronic disease management plan by the patients, and obtains complete monitoring data based on the active monitoring, which provides a basis for the adjustment of the chronic disease plan. The system has been tested to be stable and smooth, operable, user-friendly. CONCLUSION Empowered by information construction, Palm Medical Management is expected to improve the prevention and control of chronic diseases, and provide support for the realization of a modern chronic disease co-management service system that is full-fledged, intelligent and age-appropriate. |
