摘要:

目的 介绍重庆医科大学附属儿童医院前置审方规则的制订方法及应用实践。方法 医院前置审方系统上线前,审方药师基于系统规则静默运行数据进行数据清洗,并基于儿科用药多源证据的探索,构建具有儿科特色的审方规则库,建立规则维护与优化流程,从制度和流程保障儿科用药审方规则的持续改进。结果 规范运行后2023年门急诊处方和住院医嘱合格率均达95%以上,临床满意度98.7%,处方/医嘱开具错误的药品不良事件上报例次显著降低。结论 基于多源证据的儿科审方规则的合理设置和不断优化有助于药师准确审方,提高药师审方效率,规范医生开嘱行为,提高临床合理用药水平。

Abstract:

ABSTRACT: OBJECTIVE To introduce the formulation method and application practice of the prescription review rules in Children""s Hospital of Chongqing Medical University. METHODS Prior to the launch of the hospital""s prescription review system, the pharmacist silently runs data cleaning based on system rules, and explores multi-source evidence of pediatric medication to construct a review rule library with pediatric characteristics. A rule maintenance and optimization process is established to ensure the continuous improvement of pediatric medication review rules from a system and process perspective. RESULTS After standardized operation, the qualification rate of outpatient and inpatient prescriptions and medical orders reached over 95% in 2023, with clinical satisfaction rate of 98.7%. The number of reported adverse drug events with incorrect prescription/medical orders significantly decreased. CONCLUSION The reasonable setting and continuous optimization of pediatric prescription review rules based on multi-source evidence can help pharmacists accurately review prescriptions, improve the efficiency of pharmacist review, standardize doctor""s prescribing behavior, and improve the level of clinical rational drug use.
